One of the most common questions we hear from new patients is, “Why did my friend receive different recommendations than I did?” It’s a fair question, and the answer is actually one of the most important parts of practicing good medicine.
Medical marijuana isn’t a one-size-fits-all treatment. Even when two patients share the same diagnosis, their symptoms, medical history, lifestyle, and treatment goals may be very different. That’s why experienced physicians develop individualized recommendations instead of following the exact same plan for every patient.
Understanding why recommendations vary can help patients feel more confident during their medical marijuana evaluation and better understand how treatment decisions are made.
Every Patient Has a Different Medical History
A diagnosis is only one piece of the puzzle.
For example, two patients may both suffer from chronic back pain. One patient may have recently developed pain after a work injury, while the other has lived with degenerative disc disease for more than 15 years.
Although both patients experience back pain, their previous treatments, current medications, overall health, and daily limitations may be completely different. These factors help physicians determine how medical marijuana may fit into each patient’s overall treatment plan.
The same concept applies to conditions such as anxiety, PTSD, insomnia, cancer-related symptoms, neuropathy, and many other qualifying conditions under Florida’s Medical Marijuana Program.
Symptoms Often Matter More Than the Diagnosis
Even patients with the same medical condition don’t always experience the same symptoms.
Consider two patients diagnosed with PTSD.
One patient may struggle primarily with severe insomnia and recurring nightmares. Another may function well during the day but experience anxiety in crowded environments.
Although both patients have the same diagnosis, their treatment goals are very different.
This is one reason physicians spend time discussing symptoms rather than simply reviewing a diagnosis. Understanding how a condition affects daily life helps guide individualized recommendations.
Previous Treatments Help Guide Recommendations
During a medical marijuana evaluation, physicians typically discuss what treatments patients have already tried.
Questions often include:
- Have prescription medications been effective?
- Have you tried different routes of medical marijuana and found you respond differently?
- Were there unwanted side effects?
- Have physical therapy or other conservative treatments helped?
- Have symptoms remained stable or continued to worsen?
These conversations provide valuable context.
Someone who has already tried multiple treatment options without meaningful improvement may have different needs than someone exploring medical marijuana for the first time.
Different Routes of Administration Serve Different Purposes
One area that often surprises patients is that medical marijuana is available in several different forms.
Depending on a patient’s symptoms and physician recommendations, treatment may include:

These conversations allow physicians to recommend treatment plans that are practical as well as medically appropriate. If a patient wants the relief of THC for example, without the “high”, a topical product may be a great option for them compared to inhalation or flower.
Medical Marijuana Recommendations Can Change Over Time
Another common misconception is that recommendations never change after the first appointment.
In reality, medicine is an ongoing process.
As patients gain experience with treatment, physicians often review:

- Side effects
- Changes in medical history

There is rarely a one-size-fits-all approach, and recommendations often reflect both the patient’s individual needs and the physician’s clinical experience.
At Compassionate Healthcare of Florida, our physicians have evaluated thousands of medical marijuana patients throughout Florida. Based on that experience, we generally provide recommendations that give patients the flexibility to explore the treatment options available under Florida’s Medical Marijuana Program and determine, with their physician’s guidance, which routes of administration provide the greatest benefit.
For many patients, our standard recommendations utilize Florida’s current state limits of 24,500 mg of THC across non-smokable routes per 70 day period and 2.5 ounces of smokable flower per 35-day period. This allows patients to work with their physician to identify which products and routes best fit their treatment goals, rather than being unnecessarily limited from the beginning.
As treatment progresses, recommendations can be adjusted based on the patient’s experience. If additional milligrams or increased amounts for a particular route are medically appropriate, our physicians will submit a Requests for Exception (RFEs) to the State of Florida. Likewise, milligrams can be reallocated between routes as a patient’s treatment plan evolves.
Whenever possible, we also prefer to leave at least some availability in each route of administration. This gives patients the flexibility to try different treatment options in the future without unnecessarily restricting access if their needs or preferences change.
Your Medical Marijuana Orders May Temporarily Return to the State Standard Limits During Renewal
One of the most common questions we receive is why a patient’s medical marijuana orders appear to have decreased after a renewal, even though they previously had a Request for Exception (RFE) approved.
The reason is simple: medical marijuana certifications do not automatically carry over when they are renewed in Florida.
Each time your physician renews your certification, a new recommendation must be entered into the Florida Medical Marijuana Use Registry (OMMU). If you previously had an approved Request for Exception (RFE) that increased your allowable milligrams or smokable flower, that request must also be submitted and approved again by the State of Florida.
While the state allows up to 10 business days to review RFEs, approvals are often completed much sooner.
What Happens While My RFE Is Pending?
Until the State of Florida approves your new Request for Exception, your certification will temporarily reflect the standard state limits for each route of administration, along with Florida’s current aggregate limit of 24,500 mg of THC across non-smokable routes.
This temporary reduction is a normal part of the renewal process and does not mean your physician forgot to request the increased amounts.
What Happens After Approval?
Once your Request for Exception is approved by the Office of Medical Marijuana Use (OMMU), your certification will automatically update to reflect the recommendations requested by your physician. Your increased route-specific limits and any additional approved amounts will then become visible in your OMMU profile. The state will also email letting you know of the approval.
We Monitor the Process

If you’ve recently renewed your certification and notice your available milligrams are lower than expected, don’t panic—it is often simply because your RFE is still awaiting state approval.
