Amount & Tier Impact: 10% off (Green & Silver Tiers) | 15% (Gold Tier) | 20% off (Black Tier) Eligibility: Valid for use (1) time during the month of the patient’s birthday. The order must be completed within that same month. For example, if a patient places an order using their birthday discount three days before the end of their birthday month, the order must be completed within those three days.
